The incident and complaint form is for staff, students, and parents of Santa Fe Public Schools. We appreciate you taking the time to fill this out. Santa Fe Public Schools believes in creating safe environments for all students, and this is one important step in that process.   
  • Once completed, this report will be sent to the school's principal for follow up. 
  • In order to help you best, we ask that you provide as many details as possible.  
  • If the behavior has happened multiple times, you will be prompted.
